What is it? Advanced care planning is the process of preparing for decisions about your health in case you are ever unable to speak for yourself.
Advanced care planning involves reflecting on your values and having meaningful conversations about your healthcare wishes with the people you love. Putting your preferences in writing helps guide your loved ones in honoring your wishes.
Who should have an Advanced Directive? It’s recommended that anyone over 18 have an Advanced Directive, a legal document naming who they would want to be their spokesperson if they were unable to make decisions for themselves in a healthcare emergency, or due to a serious illness. You don’t need an attorney to complete an Advanced Directive, and the process is free.
